Why is your account deactivated? Post the Performance Notification you received.
Did Amazon ask you for a Plan of Action?
You're not speaking to Amazon and can't attach anything here.
That's not going to happen until you give Amazon what they want.

Plan of actions isn't accepted much anymore for most violations.
The appeal of an account deactivation needs to be done through Seller Central. You can access the Submit New Information button from the Account Health Page.
My suggestion is usually to go to the Performance Notification from when the account was deactivated and see what was requested at that time.
If you do post a Case ID or a Performance Notification, I may be able to offer some greater assistance.
