I feel that if you keep your account in good health, get your orders out within 1 day, includes putting the order together and shipping it out, you should at least be given the "privilege" of having an "Add To Cart" button.
Instead, Amazn wants you to pay an additional $40 to get this.

Every item has an Add to Cart button. How else are buyers adding it to their cart and purchasing? Do you mean you need to click on "See all offers" to see yours?
Paying the $40 a month does not guarantee you will show up on the main page. I have many items that a buyer needs to know to click "See all editions" to see my copy. They usually don't.

It is a mystery shrouded in fog and information clouded with misinformation and ALL of it is on purpose.
Amazon will favor themselves and FBA sellers virtually 100% of the time because $$$$$ talks and THEY pay Amazon more.
Amazon will NEVER give a 'formula' or every con artist on the site would have the 'buy box' and the huge majority would be their friends offshore who specialize in black hat ops and hijacking ASINs.

From Amazon's viewpoint, I can imagine that it looks like this:
Either you are selling enough, and serious enough about being a real business, that you are paying the Pro fee (which pays for itself if you sell 40 items/month), at which point Amazon might decide to give you the BB.
or
You are such a low volume seller that Amazon cannot track your customer service to the level that they feel confident recommending you to their customers.
