Replacement item??
For the first time ever, a buyer is requesting a replacement item through Amazon for a book he bought. Generally, as a seller of used books, we have only one copy of each title in stock, and cannot provide replacements. Why did this happen? What are the consequences? The "replacement" request is sitting in our Pending Orders, and I'm not certain what to do.

The official replacement option (as opposed to a buyer just messaging you with a request) is not supposed to work unless the seller has a quantity greater than one. Most sellers of used books have just one copy for an SKU, and don't see those replacement orders.
Good luck with this.

We never send a free replacement even though the buyer requests one. Too easy to be stung on the Amazon site.

Try this:
Dear Buyer,
I am sorry that the item you received did not meet your expectations.
Unfortunately, I am out of stock on the item and cannot send a replacement.
Please feel free to return the item using Amazon's return policy as shown on your packing list and on the order section of your account.
Once I receive the item back, i will issue the appropriate refund.
Regards,
Seller
The item will remain in "Pending" status until the original order is returned to you. Once it arrives, you can issue the refund and claim "Item not in inventory" for why you did not send a replacement.
