ADD products via upload: Status "FAILED"

I tried to upload the product but my file was not completed. They are always in the "Failed" state. Has anyone had this problem and can help me?

I've never actually seen it show failed before. What is your process for creating these files? Are you downloading the templates from Amazon, filling them out in Excel without modifying any of the header columns, and then trying to upload them, or are you using a different spreadsheet program and making edits to the header rows?
