- Date of supply: The time period (month and year) on which the products were supplied to your business. This period should fall maximum 180 days before the claimed period (the period where you claim to be a distributor).

- Products supplied: A list of all products/ASINs, supplied with a detailed and accurate description from your upstream supplier as listed on the government portal

- Supplier’s EPR number: Your upstream supplier’s unique identification number for EPR

- Supplier’s EPR number registration date: Your upstream supplier’s EPR number registration date, or official registration document from PRO stating the same

- Supplier’s SIREN or SIRET numbers

- Supplier's legal name: The legal name under which your upstream supplier operates and is registered

- Supplier's VAT ID: Your upstream supplier's VAT identification number or tax identification number

- Seller's legal name: The legal name under which your business is officially registered

- Seller's VAT ID: Your VAT identification number or tax identification number

We request that you present the confirmation statements for each product that’s supplied to you in the relevant time period (date of supply). You may use the attached templates to structure the required information. Once we receive this information, we'll be able to continue researching this matter.
