Yes, this keeps happening to me. Is it crazy to think that Amazon shouldn't be allowed to charge us fees on FBA orders when they completely screw up the order?

Happens all the time. I find that customers are generally more understanding once I explain that their order is late because of Amazon's incompetence, and that reaching out to them to find out why they are failing on their fulfillment promise just leads to the same basic response. Basically "Wow that sucks for you! Good luck"

Happens all the time. And I'm pretty sure we shoulder the returns costs since most are already shipped and then refused due to missing delivery date

Maybe You should ship your orders FBM instead of FBA!
