Unable to Create Plan of Action for Seller Fulfilled Prime

When trying to submit the POA for SFP. I am unable to click the "Send POA" button.
I have already attempted deleting cache, cookies, etc., in addition to trying two different browsers with fresh installs. The button is unable to be clicked at any point.
Anyone else experience this?

Not sure if I understand correctly. You lost seller fulfilled prime? If so, I do NOT believe you can appeal that.
