My account is deactivated due to multiple accounts policy violation

I wanted to share something that happened with my Amazon seller account. A while back, someone approached me and claimed to be from Amazon. They offered to help me set up my seller account, and I trusted them. They used their laptop to create the account for me.

Later, Amazon suspended my account because they thought I was operating multiple accounts. It turns out the person who helped me might have linked my account to theirs or used their own information during the setup.

When I tried to appeal, the person pressured me to lie to Amazon and say that the other account belonged to a friend who was helping me. I did what they said, but Amazon denied my appeal because the information wasn’t true.

Now, Amazon is asking me to submit a signed affidavit explaining what happened. I need to write a statement about the situation and get it notarized.

Do any one have any experience with this or know where I can find a sample affidavit? Amazon is very strict, and I want to make sure I do this correctly to get my account back.
