Ongoing Issue with OTP-Based Return Deliveries – No Resolution After 4 Incidents

I am a seller on Amazon and have been facing repeated issues with OTP-based return deliveries for canceled orders and returns initiated by buyers. Over the last few weeks, I have encountered the same issue four times with no resolution, and I am extremely frustrated by the lack of action and support from Amazon.
Here’s what happens:
The Amazon delivery agent arrives with multiple return packages. Let’s say he has 12 packages to return. He delivers 11 and says that he missed one and will bring it the next day. He takes the OTP for the 11 packages and marks them as delivered correctly.
The next day, the same agent (or sometimes another) asks for the OTP again, claiming to deliver the one remaining package. Later that day, I receive an email that the package was marked as delivered using the same OTP, but the package was never handed over.
When I contact the delivery agent, he claims he only delivered what was physically given to me. I even reached out to the agent whose ID was used for the final package delivery, but he denies any involvement and says he doesn't operate in my location.
This has now happened four times.
Each time, I contacted Amazon support and raised a SAFE-T claim, but the response has been disappointing. The claim was denied with the reason that the package was delivered via OTP. I've also raised the issue with local delivery station managers, but they have been equally unhelpful.
As a seller, I am left with no solution and no reimbursement, even though I have done everything by the book. It is incredibly frustrating to deal with a system where no one is willing to take responsibility, and the problem keeps repeating.
I want to know if anyone else has faced similar issues and what steps they took to resolve this. I am looking for advice on how to escalate this matter further, as neither Amazon customer support nor the local delivery team seems to acknowledge or help with this problem.
