How does the transparency program keep other sellers from selling your counterfeit products via FBM? If Amazon never sees the products that are shipped straight from the FBM seller to the customer, how will Amazon ever know whether there is a transparency code attached to the item?

It doesn't, it is the seller's responsibility to do it, if you are the rightful owner of the product.
You take your USTPO registration documentation and other legal documents for the product (patients, COC's, etc.) showing you are the actual product owner and send them to Amazon's Legal office in Seattle by certified mail. They will straighten things our is a week or so.
If you don't have the legal documentation, usually you are out of luck and nothing will change.

They lay this pretty explicitly at least 10+ different times during the 5 step <30 minute Seller University course/training on the Transparency program.

The orders come in with a transparency badge to let you know it is a transparency order. Before any FBM 3P seller can confirm shipment, they must provide the unique-matching transparency code.
seller generates a code for a unit of an ASIN and applies the related label to box before shipping. This code has to be provided on the manage orders page in order to list/fufill these offers.
One would have to put in the code on that specific order that matches that specific Brand+ASIN+unit before confirming the order. If no code, or an invalid code is entered, you cant confirm shipment and your offers will be removed from the ASIN.
