Refund to buyer by reason - Order not received

I need some clarification regarding an issue with an FBA order. A buyer claimed 'Order not received' and received an automatic refund, but the product was fulfilled and delivered by Amazon through FBA. Could someone help me understand why this might happen and if there’s a way to prevent it in the future? Thank you!
