Are you referring to inventory that you sent that UPS lost? If that is the case, you as the seller need to provide that.

According to the UPS optional coverage section here, Amazon doesn't provide documentation for claims - instead, they've set up a specific process with UPS for sellers:

You'll want to call the UPS Preferred Claims team at the 1-800 phone number you find in that help page. The help page mentions that these agents are specifically "trained to support claims requested by Amazon sellers." When you call, just let them know you're an Amazon seller filing a claim for lost seller-fulfilled inventory.
