I can understand your situation as I am also facing the same issue. I don't know what is going on in Amazon warehouse. I have noticed that the orders received empty parcel or wrong product received are those orders which are not received by the buyer and rejected by buyer. It means that buyer never receive the order and product was misplaced or changes by pick up team or warehouse associates.
I have also noticed that when Amazon denies the claim with threshold breached reason at that time you get the bulk quantity order and again these orders will marked as rejected by buyer and when we receive the return then it is shocking for us as product is the different item or empty box.
I think there is something wrong going on with Amazon which Amazon is noticing for now however it will impact on Amazon as day by day sellers are planning to leave Amazon and moving to other platform.
This is my total experience with Amazon now and if you would like to add on something then I will surely check out.

If Amazon doesn't find the order as fake after you fill the above form. Then do these steps below
1. If the order is COD and delivery partner is not Amazon Easy ship (3rd Party Logistics). Not Easy ship means - the Amazon Easy ship team will pick up from you, but in some cases, the parcel may handover to other logistics, ( you can see " Hand Over Address" on the shipping label ) Then DO NOT SHIP. Bear the cancellation charges. 99% it might be fake.
2. 1% of fake buyers uses Prepaid orders. for this issue: Check the delivery address, if you feel incomplete , for example ( Near temple, near square park - as the first address) - DO NOT SHIP - Bear the cancellation charges.
Now Amazon will only grant 8% of return orders. For example : if you got 1000 orders and you shipped all , later 100 orders returned to you, you can only claim 8 orders out of 100 order. If you claim more than 8% of total rreturned orders, they will reject, or wait 15 days to restart the cycle, but you have to claim within 15 days of the date of return.
Disclaimer : Keep this in mind, All information has been share from my experience and its all upto you to ship or not. I/We cannot take responsible for the cancellation charges for your cancellation.
Most importantly, do self ship: use your own 3rd party logistics, you will not get fake orders. But the con is Late delivery. Amazon has a rating for OTDR ( On time Delivery Rate) if you have late delivery , you will loss eligibilityy for few pincodes until your OTDR is above 97%.
